Click:

1. Request

2. You can now download the form.

3. PDF will appear.

(If the PDF is not ligible,and shows only boxes, this means the filling of your app is not supported.  Simply look in the top, right hand corner for a tab

4. Click "Open With A Different View", 

5. Last, Click "OPEN WITH ADOBE ACROBAT (default)".
